TRANSCRIPT & HS DIPLOMA REQUEST

Below you will find the Transcript/HS Diploma Request Form. Please make sure to read and answer all the sections of the form to the best of your knowledge. If you are unable to complete the form online or encounter any issues, you may visit or call the Main Office for assistance. Keep in mind the office is open Monday-Friday from 7:30am-4pm (more info below).

 

NOTE: For those who graduated or attended Arleta HS from 2009-2013, we no longer provide transcripts/diplomas. You may request your records at the district website.

 

Transcripts are $5 per transcript. Diplomas are $20 per diploma. As supplies are limited, payment for a diploma must be received before new diploma is created. You may pay through our online student store. Keep in mind, the online student store will charge a .55 cents fee per transcript and a $1.15 fee per Diploma. Cash transactions can be made in person at the Main Office. The Main Office will not accept any bills over $20. 

 

PICKING UP REQUESTED DOCUMENTS:

- When picking up your requested documents, bring a current photo identification.

- Diplomas cannot be mailed and must be picked up in the Main Office.
